고시 제2019-93호(약제) 경장영양제(품명: 엔커버액 등)

■ 고시 개정 전체내용
1. 허가사항 범위 내에서 아래와 같은 기준으로 투여 시 요양급여를 인정하며, 동 인정기준 이외에는 약값 전액을 환자가 부담토록 함.
- 아    래 -
  가. 경구 영양 섭취가 충분하지 않거나 경구 영양시 소화기장애(구토, 설사 등)등으로 적절한 흡수가 되지 않는 경우(예시：공장루, 위루 설치 환자(Feeding jejunostomy or Feeding gastrostomy) 등)
  나. 정맥영양요법(Parenteral nutrition)을 받고 있는 환자가 소화 기능 회복 단계에서 소량 필요한 경우
  다. 고칼로리 수액의 적용이 곤란한 환자의 경우(광범위 열상 등) 
  라. 크론병으로 확진된 입원 환자의 경우

2. 허가사항 범위를 초과하여 아래와 같은 기준으로 투여하는 경우에도 요양급여를 인정함. (엔커버액에 한함)
- 아    래 -
  ○ 관해 유도가 필요한 중등도∼중증 소아청소년 크론병환자 (PCDAI: 30 이상)에서
   - 완전장관영양법(Exclusive enteral nutrition)으로 2주 투여 후 증상이 호전된 경우(PCDAI가 25% 이상 감소 등) 최대 8주간 인정함.

   ※ PCDAI(Pediatric Crohn‘s Disease Activity Index)
