고시 제2019-21호(약제) Apixaban 경구제(품명: 엘리퀴스정 등)

■ 고시 개정 전체내용
허가사항 범위 내에서 아래와 같은 기준으로 투여 시 요양급여를 인정하며, 동 인정기준 이외에는 약값 전액을 환자가 부담토록 함.
- 아     래 -

 가. 선택적 고관절 혹은 슬관절치환술을 받은 환자 중에서 다음 한 가지 이상에 해당하는 경우 
- 다    음 -
   ○ 비만(BMI≥30kg/㎡), 에스트로겐 치료, 하지정맥류, 고령(60세 이상), 장기간의 부동(1주일 이상의 침상안정이 필요한 경우), 울혈성심부전, 호흡부전, 악성종양, 중심 정맥 카테터 삽입, 항암화학요법, 중증감염증, 정맥혈전색전증의 과거력
 나. 비판막성 심방세동 환자에서 뇌졸중 및 전신 색전증의 위험 감소
   ○ 비판막성 심방세동 환자 중 다음의 고위험군에 투여시 인정
- 다    음 -
    1) 뇌졸중, 일과성허혈발작, 혈전색전증의 과거력이 있거나 75세 이상 환자 또는
    2) 6가지 위험인자(심부전, 고혈압, 당뇨, 혈관성질환, 65-74세, 여성) 중 2가지 이상의 조건을 가지고 있는 환자
 다. 심재성 정맥혈전증, 폐색전증의 치료 및 재발위험 감소
   ○ 투여기간
    1) 급성 심재성 정맥혈전증 및 폐색전증 치료 초기: 7일 이내
    2) 심재성 정맥혈전증 및 폐색전증 치료(초기치료 이후 유지)와 재발 위험 감소: 6개월 이내
단, 특발성, 원인불명인 경우의 재발, 위험인자가 교정되지 않는 경우는 지속투여를 인정함.

■ 고시 개정 사유
교과서, 가이드라인, 학회의견 등을 참조하여 심재성 정맥혈전증(DVT)와 폐색전증(PE) 환자 중 6개월 이상 지속투여가 필요한 환자에 대한 급여를 확대함.
